The Catholic Family Awareness Program is a ministry to educate families, to promote discussion between family members and to provide a booklet entitled Catholic Family Record Guide.
The purpose of this booklet is for you to record information so that at the time of death, loved ones do not have to make difficult decisions without indication of your specific wishes.
